CenterLight Health System’s history traces back to 1920, when our then parent company was established as a nursing home serving impoverished New Yorkers. Today--100 years later--we have evolved into a completely different organization, to address the contemporary healthcare needs of those entrusted to us.
However, our mission of helping New York seniors remains steadfast. Teamcare, CenterLight's Program of All-Inclusive Care for the Elderly (PACE), is New York’s largest PACE plan and helps individuals 55 and older continue to live at home and in the communities they love. We want our seniors to live a healthier and more fulfilled life in the comfort of their home and the familiarity of their community. Teamcare offers the largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the nation, with 14 centers located throughout the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Suffolk and Westchester counties. We offer medical care, social supports, activities and events, opportunities for socialization, physical, occupational and speech therapies and other services on site.
